Months before his death, Alan Thicke received an ominous warning about his heart.

The late Growing Pains star died Dec. 13 at age 69 after collapsing while playing hockey with his youngest son, Carter. His cause of death was later revealed from a "ruptured aorta" and a "standard type A aortic dissection," according to his official death certificate obtained by PEOPLE.

Before his passing, the actor and his wife Tanya (Callau) Thicke filmed an episode of Hollywood Medium with Tyler Henry, set to air on Wednesday. In a sneak peek at the episode shared by E! News, Henry meets with Tanya to watch Tyler's reading with Alan, in which he warned the actor about the health of his heart months before his death.

"I want to keep this in mind … I have to talk about health, and it's kind of health with family," Henry tells Alan in the clip. "For the men in your family, it's really good just to consider that blood pressure is going to be something we must keep in mind."

"When it comes to like a family and a gene perspective, it's possible that within your family there might be multiple men who end up at a later age dealing with blood pressure issues, but also deal with either a heart murmur or heart arrhythmia," he continues. "But I feel like I have to go to heart, which correlates with blood pressure."

"So kinda keep that in mind. I have some people who came through and acknowledged passing similarly," says Henry. "It's almost saying like, 'Keep in mind your own heart.'"

Alan then receives a message telling him to "be proactive" about his health.

In an interview with E! News this week, Tanya, who married Alan in 2005, said it's "devastating" to know that Henry sensed something was wrong with Alan's heart.

"The fact that he touched on the heart and kind of kept emphasizing that — my husband went in for a full checkup two months later, and he had a clean health of bill," she said. "What happened to Alan is a massive heart attack — an aorta. It's hereditary, and there was no indication that that was in the family histories. It wasn't something that the doctors would have been looking for."

She also revealed the two were preparing to add a baby to their family.

"Alan went in for a full physical because we were planning on having a baby, so to go and do the whole sperm thing, he wanted to get a full physical and make sure that he was healthy and ready to go … the timing of it was just unbelievable," she said.

"Tyler did say — this is something that's uncovered on the episode — but when you did your reading, Alan did ask him, 'Do you see children?' And Tyler said, 'No,' " she continued. "And I kept saying to Alan, 'Isn't it strange that Tyler didn't see that?'"

Tanya is in a legal battle with Alan's two eldest sons, Brennan and Robin Thicke. The two filed a petition against her last week, claiming in documents obtained by PEOPLE that she is now alleging that the prenuptial agreement she signed ahead of marrying the actor is invalid.

However, Tanya's attorney Adam F. Streisand told PEOPLE Tanya "has never claimed that the prenup is invalid and has no intention of ever claiming it."

"Tanya Thicke has never threatened to take private family matters public, and she never has," Streisand said in a statement to PEOPLE. "It is clear that Alan's sons have chosen this distasteful public smear tactic to bully Tanya by stirring up the tabloid media, filing a bogus lawsuit, and refusing family mediation. Tanya is still grieving the death of her beloved husband and, out of respect for Alan's memory, intends to handle his sons' false statements privately."

Hollywood Medium with Tyler Henry airs Wednesdays (8 p.m. ET) on E!
